Adoption Agreement

Initial Wellness Guarantee:
Clover Patch Puppies guarantees that the puppy has age-appropriate vaccinations and deworming, and is in good health to the best of our knowledge at the date of this agreement. Clover Patch Puppies will provide a record of the puppy’s vaccinations and deworming. Within seven days of the buyer taking possession of the puppy, the puppy must be examined by a licensed veterinarian (of the buyer’s choice) for a wellness exam. Failure to have the puppy examined in this time frame will void the initial wellness guarantee. 
If the veterinarian finds any reason the buyer should return the puppy at this initial wellness examination, Clover Patch Puppies will provide a full refund to the buyer of the purchase cost of the puppy contingent on the return of the puppy within 7 days of the initial examination and documentation from the veterinarian. Any travel and/or veterinarian expenses incurred by the buyer will not be refunded by Clover Patch Puppies.

Rehoming Agreement: 
If the buyer decides they can no longer care for the dog/puppy for whatever reason the buyer agrees that they will contact Clover Patch Puppies. Clover Patch Puppies will welcome the dog/puppy back at any time. 

•  If the puppy is returned within 30 days of pick-up a 50% refund of the purchase cost of the puppy will be issued to the buyer.  Otherwise, a refund is not guaranteed.

• If the buyer wishes to rehome the puppy/dog to a friend or other individual. The new owner must be approved by Clover Patch Puppies and sign our rehoming agreement.
